First Day of School REQUIRED DOCUMENTS

STEP 1 - Please DOWNLOAD all the following documents below.

STEP 2 - FILL them out on your computer electronically and PRINT A COPY of each one out.

STEP 3 - Then, SIGN IT and RETURN the signed copies (7 forms in total) to your teacher on your child’s first day of school.

Please note:  We only accept a print copy with your actual  ”pen signed” signature on it.

(NO electronic files will be accepted – DO NOT email documents to the school)
